About the Job
Family Consultant is to provide consultation, care planning assistance and support services to clients and their families.
This is an onsite opportunity in the Fresno/Clovis area for a non profit organization. There may be a possible need in Modesto also. Primary responsibilities include:
- Consult with family to perform care planning consultation, education and support services.
- Conduct a formal assessment of needs, develop short-term and long-term care plans.
- Provide referrals to other community agencies, and authorization of appropriate services.
- Oversee respite program care services, make formal assessments of eligibility, home visits; monitor the quantity and quality of services needed and received. Resolve or mediate problems that arise.
- Collect data on all facets and types of assistance provided to families.
- Organize and participate in educational programs as needed.
- Represent the agency through public speaking engagements, training activities, community outreach efforts and at community meetings.
- Carry out activities as required by Grantors/grants and report grant related activities to the Program Manager.
QUALIFICATIONS
Education
- Bachelor's Degree in Social Work, Psychology, Sociology, Counseling, Gerontology or related field preferred
Skills
- Minimum 2 years' experience in Case Management, Gerontology or a related field.
- Bilingual skills strongly preferred.
- Experience in a non-profit organization preferred.
- Knowledge of counseling and social services principles and practices.
- Knowledge of adult learning principles, with ability in public speaking and training
