Est. Pay Range: $68.00/Hour - $74.00/Hour
Candidates must be licensed in at least four of the following states. Can be any combination of the states below, does not have to be specific to any POD. The more licenses the better. Please list licensed states at top of resume.
POD 1: DC, DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, VT
POD 2: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV
*IF* candidate is licensed in one of the states BELOW, they must hold IP in that state. If they don't hold a license in one of these states, they don't need IP.
Here are the requirements to Independent Practice. Please make sure the state(s) they hold IP in are clearly noted on their resume.
AR 6240 hours
CT 2000 hrs and 3 years with any CP; other state collaborating hours do qualify
FL 3000 hours of overall experience (100 wks or 2.5 yrs)
IL 4000 hrs (133 wks or 3 yrs of overall practice)
KY 4+ yrs of prescribing experience overall
MA 2+ yrs experience
ME 24 months of supervised practice
MN 2080 hrs of experience
NE 2000 hrs of overall experience
NY 3600+ hrs of overall experience (120 wks or 2.5 yrs of practice)
SD 1040 hrs of experience
VA 3 yrs of overall experience
VT 2400 hrs of overall experience
WV 3 yrs of overall prescribing experience
The Telehealth Nurse Practitioner or Physician Assistant (Provider) delivers patient care services through a remote technology platform. You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above.
Encounters are documented utilizing an electronic health record (EHR). Telehealth providers report directly to the Enterprise Initiative Lead.
1. Patient-Centered Quality and Safety
• Evaluate primary care, acute, chronically ill, and transitional care patients, in addition to providing healthcare education and counseling, and disposition planning for our patients ranging in age 18 months and above
• Provide patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
• Educate patients on health maintenance and respond to patient care inquiries
• Document all patient care within an EHR according to MinuteClinic policies and procedures
• Provide care and coordination of our patients with internal and external colleagues, including the broader patient centered medical home, ensuring the highest standard of care is provided for all patients and at all times
• Effectively work within a patient care team, including fellow Providers, Collaborative Physicians, para-professionals, Pharmacists and other members of the health care team
2. Healthcare Environment Management
• Resolve conflict using appropriate management techniques
• Cultivate and maintain positive relationships among practice employees, CVS Health colleagues and external partners
• Reprioritizes continually throughout the day to fulfill patient and business needs
• Support the overall patient experience, by effectively managing clinical and non-clinical duties as well as patient expectations
• Quickly adopt new service offerings and patient care models
• Adhere to the core values of CVS Health MinuteClinic in all communications and interactions
• Assist with hiring, development, and evaluation of Practice employees
• Complete necessary tasks for clinic operations, including but not limited to taking inventory, following up on lab results, receiving incoming phone calls, ordering supplies and maintaining clinic cleanliness
Must be willing/available to work two weekends per six week cycle
Must be willing/available to work one major holiday and one minor holiday per year.
Completion of a Masters Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role required.
10:30pm-6:30am, including weekends and rotating holidays, appx 20 hrs/wk
