Title: Field Based Nurse Practitioner
Location: Philadelphia, PA
Job-Type: FTE/Permanent
Duties & Responsibilities:
• Perform home visits with patients enrolled the program. Initial visits are comprehensive evaluations with subsequent visits targeting risk factors for disease exacerbations and hospitalizations.
• Prescribe medications, order labs or diagnostic testing, and make sub-subspecialty referrals collaborating with the community PCP. Interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions
• Educate patients and/or patient's family on chronic medical conditions, preventative care, and medication adherence compliance
• Coordinate with patient's other providers including PCP, specialists and health plan case managers to ensure all are working together with the patient to achieve the same health goals. Work closely with the interdisciplinary team, which includes, social work, nursing, pharmacy, palliative care, pharmacy, collaborating physician, and behavioral health.
• Share after hour on-call coverage with other providers on the team. Limited weekend on call responsibilities.
• Clinical documentation of acute and chronic healthcare issues through patient encounter in EMR system done in a timely manner. Work with documentation and coding team
• Participate in virtual clinical meetings, such as rounds and POD huddles, and online trainings. Attend in person meetings as needed and scheduled.
Qualifications:
• Graduated from an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Medicine
• Active, unrestricted NP license in Pennsylvania
• Ideally, at least 1 or more years of NP experience in Primary Care, geriatric, adult, home care, or family medicine (new NP grads may be considered with the right RN experience)
• Must have current BLS certification; ACLS preferred.
• Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies
• DEA license and State Controlled Substance License upon hire or within 3 months of hire;
• Electronic Medical Records experience as well as strong computer skills (MS Office Suite)
• Strong background/interest in home based primary care and preventative medicine
• Demonstrated ability to identify, diagnose, treat and document acute and chronic disease in adults, and ability to recognize conditions, characteristics, and behaviors that place patients at risk.
• Must have valid state driver's license
• Highly motivated, self-directed professional with strong organizational skills and comfortable working independently
• Effective leadership and interpersonal skills; facilitates problem resolution and maintains a professional demeanor in difficult situations.
• Proven ability to pursue feedback, continuing education and professional development
• Proof of yearly influenza vaccination, Hepatitis B vaccination, and TB testing required.
