Overview
Halifax Health - Hospice is seeking a Field Certified Nursing Assistant.
Summary
The Field CNA provides personal care and light housekeeping tasks to hospice patients in a variety of settings (Homes, Nursing Homes, Assisted Living Facilities, Boats, etc.) per plan of care developed by the Interdisciplinary Team. Assists skilled nursing staff by providing personal care training and support needed to the caregiver. Responsible to complete daily schedule of visits.
Job Qualifications
- High School Diploma or GED equivalent required.
- Current/active Florida CNA certification required.
- Valid driver's license with a clean driving record; personal car in good working order
- Able to work well with Interdisciplinary Team Members
- Excellent interpersonal and verbal communication skills
- Excellent customer service skills to maintain positive, professional working relationships, work with a variety of individuals (patients, families and skilled clinicians from the hospital, nursing homes, ALF's, physician's offices and community)
- Ability to cope with high stress levels related to terminally ill patients/families and flexibility with frequent changes to assignments/duties based on patient needs
