Halifax Health - Hospice is seeking a Field Chaplain.
Summary:The Chaplain will be responsible for the spiritual care assessment and participate in the development of the interdisciplinary plan of care for the terminally ill hospice patients and their families, as assigned. The Chaplain will ensure the best spiritual outcomes as the patient desires or requests.
Job Qualifications:- Masters of Divinity, M.Div., required
- Member of the College of Chaplains
- One basic unit of completed CPE required
- Sanctioned by a recognized religious organization
- Previous experience in Hospice or a health care setting preferred
- Excellent time management, organizational skills, self-awareness and boundaries which are modeled for others
- Valid driver's license with a good driving record; personal car in good working order
- Provide and document spiritual care assessments
- Provide spiritual support and services to the patient/caregiver
- Facilitate contact with community spiritual support
- Assess bereavement needs
- Provide crisis intervention
- Serve as a resource to the interdisciplinary team
- Participates in education of the community regarding Hospice care
- Provides direction and supervision to assigned Spiritual Care Volunteers
- On-call rotation
