Summary:
To facilitate client access to medical and financial benefits commensurate with eligibility. To administer the agency Rep-Payee program.
Responsibilities:
Assist all referred clients in applying for and obtaining medical and financial benefits. This includes but is not limited to: accompanying client to local offices to complete necessary applications ensuring client consent is obtained in order to release documents required for eligibility determination and ensuring timely agency release of required documentation.
Develop and maintain a tracking system of the clients at risk of loosing benefits due to
spend down formulas; paid employment and lump sum settlements.
Provide assistance to the client the treatment team and the billing department in �nderstanding the benefits system generating bills and clarifying spend down amounts.
Develop a liaison relationship with all programs local welfare Medicaid food stamp and SSI offices.
Maintain a current knowledge base about the application process for all types of benefits and
eligibility guidelines.
Provide 25 hours direct service per week. This includes but is not limited to: documenting client direct service according to agency funding source and accreditation standards; case record tasks performed following agency procedures updating client database and reviewing outside correspondence.
Adhere to client confidentiality and client rights procedures.
Administer agency Rep-Payee program according to established agency policy.
Attend weekly supervision team meetings and other activities as required.
Maintain a valid chauffeur's license and the use of own vehicle in order to transport clients.
Participate on committees as designated by supervisor; attend meetings on time and actively participate in both discussion and assignments.
Participates in trainings as necessary.
Other information:
QUALIFICATION REQUIREMENTS:
Three years experience as a case manager with the psychiatrically disabled and knowledge about community resources. Bachelor's degree in Human Services preferred.
JOB KNOWLEDGE SKILL AND ABILITY:
Ability to read analyze and interpret professional journals governmental regulations.
Ability to write reports business correspondence.
Ability to effectively present information and respond to questions from groups of managers clients customers and the general public.
Ability to apply common sense understanding to carry out instructions furnished in written oral or diagram form.
Ability to deal with problems involving several concrete variables in standardized situations.
Ability to successfully complete all required training and certifications
Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.
Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
