Clinical Consulting Services (CCS), a clinician-led organization committed to high-quality patient-centered care, is seeking a Float Clinical Care Coordinator (RN MDS Assessment Coordinator RNAC).
This role supports our client’s nursing homes by assisting, substituting, and training MDS Coordinators as needed. As part of CCS, you’ll work in an environment that values curiosity, teamwork, trust, empathy, and the joy of making a difference in healthcare.
Shift: Full-time, flexible schedule
Travel Requirements: This role may involve travel between client facilities. Mileage reimbursement is available.
Primary Responsibilities:
- Clinical Support and Training:
- Assist nursing facilities and MDS Coordinators in completing resident assessments per state and facility policies.
- Conduct clinical record reviews, ensuring accurate and consistent documentation.
- Train and mentor MDS Coordinators on assessment procedures and documentation standards.
- Provide hands-on support to address specific facility needs.
- Intervention and Auditing:
- Identify areas of concern within facilities and collaborate to formulate effective action plans.
- Conduct interventions, audits, and follow-ups based on facility requirements.
- Remote and On-Site Flexibility:
- Semi-remote work available for candidates with over five years of MDS experience and Pennsylvania Case-Mix experience.
Qualifications:
- Registered Nurse (RN) licensed in Pennsylvania.
- Minimum of 1 year as an RN Assessment Coordinator experienced with the MDS process.
- Flexibility to float between multiple locations.
