This per diem position, offersan opportunity for the LVN to work under the direction of a Registered Nurse, and provide skillednursing and palliative care to patients at their place of residence. The LVNworks cooperatively with other Interdisciplinary Team members and providesintermittent visits or shifts of skilled nursing care, including painmanagement/symptom control, and nursing observations. The LVN providesinformation about the expected course of illness, care techniques and otherhealth measures to patients, family members or primarycaregivers. The LVN participates in the care plan development andimplementation. Must be able to utilize computers and be comfortablewith electronic medical records.
QUALIFICATIONS:
- Graduate of an accredited school of vocational nursing.
- Current LVN license within the State of California. Certification in Hospice and Palliative Nursing encouraged.
- At least one year of LVN experience.
- Valid driver's license, automobile insurance, reliable and well maintained transportation.
- Spanish speaking a plus, but not required
