Essential Job Functions:
- Develop and ensure quality technology minded focus on all HCBS Services for supported individuals.
- Collaborates closely with the Assistive Technology Coordinator to facilitate access to enabling technologies for individuals served.
- Collaborates with Human Resources and services leadership in team management, including but not limited to interviewing, hiring, team member placement, performance management, and termination.
- Ensures supported individuals have adequate scheduled staff coverage each month and provides coverage as necessary.
- Promotes and maintains the health, safety, and welfare of all employees and individuals of the assigned team.
- As assigned, conducts investigations, documents and reports on human service-related issues within assigned team and informs direct leadership.
- Maintains and ensures required electronic documentation is completed in a timely manner per One Vision policy.
Required Skills/Abilities:
- The Smart Living Team leader position is a safety-sensitive job. Safety-sensitive jobs or positions involve responsibility for the employee's own safety and the safety of clients and others such that an accident could cause loss of human life, serious bodily injury, or significant property or environmental damage.
- A valid driver's license is required to support persons served by providing transportation for medical appointments and activities in the community. Physically be able to appropriately bend, stoop, kneel, reach with hands and arms, walk, stand, and balance self to support persons served in completion of their activities of daily living.
- Comprehend and implement legible written and verbal instructions to complete required paperwork and support individuals in completion of their activities of daily living.
- Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook, with the ability to create and manage spreadsheets, develop reports, and deliver presentations.
- Able to see close and at a distance to complete required paperwork and support individuals in completion of their activities of daily living.
- Able to hear sounds to support and respond to individual needs.
- Ability to lift and transfer per One Vision policy, up to 50 pounds.
Job requirements:
- BA/BS degree in a human service-related field is preferred with two years of experience in supporting individuals with disabilities, or a high school education or GED with five years of experience in supporting individuals with disabilities.
- One year supervisor experience.
- Offer of employment is contingent upon pre-employment background check, and health screenings.
