OVERVIEW
Boston Senior Home Care is a private, nonprofit organization that provides case management, person-centered care planning, home care and long term services and supports to those who wish to remain in the community but require assistance to do so. One of 26 Aging Services Access Points (ASAPs) in Massachusetts, Boston Senior Home Care is committed to providing the best assistance possible for those in need. Although the majority of the people we assist are over age 60, we offer long term services and supports for children and young adults. We also provide programs and education initiatives for those serving as caregivers and information about maintaining a healthy lifestyle and controlling chronic disease at any age. We are proud of our organization, our staff, and the people we serve. We have been a part of the Boston community for more than 40 years and we love what we do.
JOB SUMMARY
Under the general direction of the Team Manager but working collaboratively with the SCO representative(s), the SCO GSSC plays an integral part of the case management functioning of the agency. The SCO GSSC will provide initial assessment and ongoing assessments of the SCO client's needs including eligibility for any relevant Boston Senior Home Care (BSHC) service and other community resources. Furthermore, the SCO GSSC will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment. This position requires the ability to work independently, multitask, conduct home visits and family/provider meetings, telephone contact and complete regular documentation. The SCO GSSC is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, SCO representatives, vendors, potential referral sources and other community agencies. SCO GSSCs actively participate in BSHC organizational life.

AN IDEAL CANDIDATE WOULD HAVE:
- Bachelor's Degree in social work, human services, nursing, psychology, sociology, or related field is preferred.
- Incumbent must be bilingual English/Vietnamese to apply for this position
- SW, LSW and relevant work experience preferred, but not required.

- Sensitivity to elder issues and commitment to maintaining clients at home with dignity.
- Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential.
- Excellent assessment and documentation skills required.
