Our Health Champions play a crucial role in helping our patients get the care they deserve by developing a deep relationship with all of our patients and their loved ones. You will do so by ensuring patients receive timely and appropriate care based on the values, preferences, and goals of each unique individual patient. Each Health Champion works as part of a multidisciplinary care team passionate about caring for older adults. The goal of the Health Champion is to help patients and their loved ones navigate the complex healthcare system by providing person-centric, age-friendly care while working with them to meet their goals.

You are excited to work alongside a dedicated care team supporting patients and will collaborate to deliver great care to patients and their loved ones with Primary Care Practitioners, Registered Nurses, Patient Services Coordinators, and a Behavioral Health Specialist. We will deliver care together by building trust, providing effective written and verbal communication, enabling each other, ensuring our patients’ voice is heard, and meeting the care needs of those we serve.
What You'll Do
- Join a small, cohesive care team that will rapidly test and refine Patina’s newest care offering: Patina Total Health.
- Outreach to patients who qualify for the Patina Total Health program to try to engage them in our model, understand their goals, and navigate them to solutions.
- Serve as the primary point person to build trusting relationships with patients and their loved ones to support every step of their care journey and aging experience. From welcoming them to Patina and learning what matters most to them, to being their key contact through phone, video, and chat for ongoing care needs, and proactively engaging them on preventative care opportunities, you’ll play an integral role in the healthcare and aging experience of your patients.
- On a regular basis you’ll assist patients and their families in meeting their goals through activities such as:
- Engaging and welcoming new patients to Patina and understanding “what matters most” to them
- Identify and motivate patients to achieve their health and aging goals
- Help patients to prepare for their medical appointments
- Act as the patient and loved one’s primary point of contact and take on legwork to support them in navigating their healthcare journey
- Respond to questions, promptly escalate clinical concerns, and provide the first line of problem solving support
- Conduct proactive outreach to help identify and address potential gaps in care
- Assess and improve patients’ motivation for achieving their care goals
- Find and connect with community resources to help overcome barriers, and advocate for our patients in obtaining solutions to barriers such as lack of transportation, language barriers, and mistrust of the healthcare system
- Provide patients with information and resources related to their health benefits, including cost of care, community/local resources, and in-network options for care
- On a regular basis you’ll be part of helping the medical practice operate smoothly through activities like:
- Schedule patient visits with our Primary Care Providers (PCPs) or behavioral health specialists.
- Document activities in the electronic medical record (EMR) and Patina’s Care Team application as appropriate.
- Participate in interdisciplinary care team meetings, providing regular updates on patient progress, barriers, and changes in status.
- Give and receive feedback to help us continuously learn and improve and strengthen our care model.
What is Required
- An empathetic and compassionate individual with a passion for delivering exceptional patient care through a team-based care model.
- Experience delivering world-class healthcare as a medical assistant, case manager, social worker, nurse, care navigator, community health worker or other direct-care roles.
- Sound critical thinking and independent judgment skills.
- Strong collaborator with proven ability to effectively communicate in writing and verbally within a team.
- Willingness to tackle any problem, large or small, to help the patient and the team succeed.
- Demonstrated resilience and an ability to remain focused and calm during challenging situations.
- Ability to learn and utilize motivational interviewing and behavior change techniques.
- Demonstrated experience navigating modern healthcare technology platforms seamlessly while providing an exceptional patient experience. Ability to use different technical modalities interchangeably (eg. computer, tablet, phone) and strong learning agility for new technologies, including our care team and patient tech platforms, as well as corporate tools (e.g., Google Suite and Slack).
- Enthusiasm about being part of a growing company with the ability to deal with ambiguity and flex within their role accordingly.
- A desire to be challenged, thirst to consistently learn, and ability to balance multiple priorities in a rapidly evolving startup environment.
What Would be Great to Have:
- Experience working in a fast-paced and swiftly growing start-up environment.
- Experience in primary care, geriatrics, or complex patient needs.

- Remote, but located in the Charlotte, NC/SC Metro area.

At Patina, our salary bands are determined by the role and career level. The range displayed on each role posting reflects the minimum and maximum target salary (total cash compensation) for the position across all locations. Within the band, individual pay is determined based on skills, experience, capabilities, work location, required licensure and certifications. Furthermore, based on our compensation philosophy, which is grounded in hiring mission-driven and talented crew members and paying competitively while ensuring pay isn’t their sole determinant for joining or staying at Patina, we generally target the middle of the range for our total cash compensation package.
The total cash compensation band for this position is $55,000 - $70,000.
