About the Job
The Healthcare Navigator serves a vital role in helping patients/consumers prepare applications to establish eligibility and enroll in coverage through the Health Insurance Marketplace, the programs in Florida KidCare, Florida Medicaid, or localized safety-net resources. This includes screening to help patients/consumers find out if they qualify for insurance affordability programs, and if they are eligible, to get enrolled. Healthcare Navigators also provide outreach and education to patients/consumers to raise awareness about publicly funded healthcare programs available to them.
Essential Duties and Responsibilities:
- Lead designated region by developing relationships within the community that is being served.
- Participate in outreach events and conduct Health Insurance Marketplace presentations.
- Conduct public education activities to raise awareness about Florida KidCare, the Health Insurance Marketplace, and Medicaid.
- Provide outreach and enrollment services as requested.
- Conduct marketing events which can include social media posts.
- Conduct patient/consumer assistance in-person and/or virtually with culturally and linguistically appropriate content.
- Ensure accessibility and usability of enrollment tools and functions for individuals with disabilities.
- Provide information and services in a fair, accurate and impartial manner.
- Maintain expertise in eligibility and enrollment through the health insurance marketplace using a range of activities such as, available community resources, learning webinars, and training opportunities.
- Serve as an ongoing resource for patients and community members.
- Maintain and update community contacts and other information to ensure cooperative working relationships with agencies and community partners interested in health insurance marketplace.
- Identify new locations to provide Navigator services.
- Have knowledge of any scheduling software used to support Navigator activities.
- Collaborate with Community Services Specialists, Community Health Education Specialist, and Community Programs & Resource Development Manager to assist patients/consumers with appropriate and needed resources.
- Conduct activities to identify interest and needs.
- Document activities and records information such as outreach completed, presentations conducted, and patients/consumers assisted.
- Ensure confidentiality and HIPAA compliance at all times
- Keep other care team members informed when situations occur that disrupts timely patient flow through site.
- Contribute to community give back initiatives and be an advocate for Premier’s mission
- Adhere to patient care needs and the core values of Premier Community HealthCare to provide best-in-class patient experience
- Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI/QA)

- Ability to work with culturally and linguistically diverse populations.
- Excellent presentation, communication, and facilitation skills.
- Well versed in public programs for healthcare, including Florida Medicaid, Florida KidCare, the Health Insurance Marketplace, and county/local resources.
- Ability to coordinate with multiple disciplines within the health center and community organizations.
- Proficiency in Microsoft Office applications and virtual applications such as (Zoom, Google Meets, MS Teams, etc.)
- Available for extended hours, occasional evenings, and weekends.

Qualifications:
- Undergraduate degree or a minimum of three (3) years of experience working with individuals and families
- Current or prior certification as a Marketplace Navigator or Certified Application Counselor (CAC) preferred
- Bilingual preferred
- Valid driver’s license and reliable transportation
- Obtain and maintain Epic certification/ annual compliance throughout employment
