High-Risk Prenatal RN Case Manager - St. Clair County, IL Government

Position Title: High-Risk Prenatal RN Case Manager                           Hourly wage: $28.00/hour

POSITION SUMMARY

With limited direction and supervision, plans, develops, and provides professional nursing services within assigned program.         Acts as a resource  and  consultant  to  community and  government agencies to provide access to care.

SUPERVISION EXERCISED

This position provides no direct supervision of Health Department staff.

SUPERVISION RECEIVED

This position is under the direct supervision of the assigned Manager and Division Director.

ESSENTIAL FUNCTIONS:

1. Assesses individual and family needs in conjunction with a multidisciplinary team.
2. Develops individualized client/family care plans.
3. Implements individual and family care plans.
4. Evaluates the effectiveness of client and family interventions towards achieving established goals.
5. Performs clinical nursing procedures.
6. Provides health education to clients, families and groups.
7. Acts as a referral/resource person in health and human services.

JOB REQUIREMENTS & QUALIFICATIONS

Certification of graduation from an accredited School of Nursing,

- AND -

A current Illinois license as a Registered Nurse by the Department of Professional Regulation.

One year of public health experience preferred.

ESSENTIAL PHYSICAL/COGNITIVE ABILITIES

• The ability to sit, flex, stand, reach, and bend.
• The physical strength to lift, carry, and transport materials/equipment equivalent to thirty (30) pounds.
• Visual acuity, auditory ability, and clear written and verbal communication ability.
• Manual dexterity and the ability for data input into various computer programs.
• The ability to maintain professional behavior at all times.

SPECIAL REQUIREMENTS

• Must reside within St. Clair County, IL or be willing to relocate to St. Clair County within six (6) months from hire date. 
• Theabilitytoworknecessaryhourstomeetthepositionrequirementsincludingeveningsand/or weekendsasassigned.
• May require provision of services at off-site locations.
• A validDriver'sLicense.
• A dependable automobileforuse in position.
• Autoinsuranceineffect with at least the minimum limits of liabilityas outlined and required by the state of Illinois.
• Adherence to all local, state and federal regulations pertaining to OSHA guidelines.
• Health Department employees:
  • Are required to have a TB test upon hire and separation of employment.
  • Must be current on CDC Healthcare Personnel Vaccine Recommendations, which include  Hepatitis B, MMR (Measles, Mumps Rubella), Td/Tdap (Tetanus/Diphtheria/Pertussis), Varicella (chickenpox) and Meningococcal if over 65. Influenza and COVID vaccines are recommended and encouraged, but not required at this time and maybe subject to change.
