We are seeking a passionate and dedicated Home Care Physical Therapist to join our dynamic healthcare team. This is a permanent, full-time position that offers the opportunity to provide one-on-one, personalized care to patients in their homes. The ideal candidate will have a strong background in rehabilitation, home care, home health, and OASIS. If you are a compassionate, patient-focused professional with a commitment to providing exceptional care, we would love to hear from you.
Responsibilities:
1. Conduct thorough assessments of patients' physical condition, limitations, medical histories, and personal goals.
2. Develop personalized treatment plans that align with patients' needs and objectives.
3. Implement therapeutic exercises and techniques to improve patients' mobility, strength, flexibility, and functional ability.
4. Educate patients and their families about the therapeutic process, promoting a comprehensive understanding of the benefits, expectations, and potential challenges.
5. Regularly evaluate patients' progress, adjusting treatment plans as necessary to ensure optimal outcomes.
6. Accurately document patients' information, treatment plans, and progress reports in accordance with regulatory requirements.
7. Collaborate with a multidisciplinary team of healthcare professionals to provide holistic care.
8. Adhere to professional standards, home care policies, and procedures, as well as federal, state, and local requirements.
9. Utilize OASIS data set to inform care planning and ensure appropriate documentation of home health services.
Qualifications:
1. Bachelor's or Master's degree in Physical Therapy from an accredited institution.
2. Current state license as a Physical Therapist.
3. Minimum of 1 year of experience in a physical therapy role, preferably in a home care setting.
4. Proficiency with OASIS documentation and other home health care protocols.
5. Demonstrated experience in rehabilitation and home health care.
6. Excellent interpersonal and communication skills, with the ability to build strong relationships with patients, families, and team members.
7. Strong critical thinking and problem-solving skills, with the ability to make sound clinical decisions.
8. Ability to manage a diverse caseload, prioritize tasks, and work independently.
9. Compassionate and empathetic demeanor with a strong dedication to patient care and wellness.
10. Reliable transportation and willingness to travel to patients' homes within the service area.
