Position Summary:
We have an exciting opportunity to join our team as a Home Health Aide - Hospice - Full Time, Days (8:30AM - 4:30PM, Alternating Days).
In this role, the successful candidate A member of the Hospice department team providing personal care services to patients of all ages in accordance with the established plan of treatment in their Homes under the direct supervision of a Hospice nurse.
Job Responsibilities:
Performs other duties as assigned
Provides personal care and or assists patients of all ages according to their abilities and the plan of care
established by the Hospice Nurse via the following activities: hygienic care, light housekeeping, preparation of
meals, feeding, exercises
Observes and communicates changes in patient's status or condition as directed by the Hospice Nurse. Sets
priorities. Completes patient care reports, forms, daily vouchers, and schedules.
Encourages maintenance of the patient's medication, supplies and equipment. Reports environmental or
equipment safety hazards to home office, patient, and family, with suggestions for improvement.
Supports and complies with all departmental and organizational policies and procedures.
Maintains caseload, schedules and travels to clients homes
Performs patient care techniques, which require manual dexterity, visual inspection, manual palpation, auditory
(auscultation); visual discrimination and physical movement of patients and equipment.
Minimum Qualifications:
To qualify you must have a High School Diploma or Equivalent. Certificate of completion of a state-approved program offered for Home Health Aides. Department of Health (DOH) approved training program. 1 - 2 years Experience working as a Home Health Aide (HHA)/Personal Care Aide in a Home Care or Hospital setting. Excellent communication (both oral and written), interpersonal, and organizational skills. Ability to negotiate architectural barriers in patients Homes, and work within the environment of care. Ability to travel to patients Homes under all weather conditions. NYS drivers license and vehicle.
Qualified candidates must be able to effectively communicate with all levels of the organization.

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $31,304.40 - $47,221.70 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
