Custom Care Home Health - Community Liaison / Marketer

As a Community Liaison, you will play a crucial role in developing and maintaining client relationships, leading to referrals and admissions. You will have the opportunity to promote our home health services and create a positive company image in the community.
Job Summary:
- Establish professional relationships with physicians, discharge planners, and other healthcare decision makers through ongoing education and successful sales strategies.
- Educate the community, referral sources, patients, and families on our hospice services through planned activities and presentations.
- Work with the Director of Business Development to develop and execute strategic marketing plans to grow the patient census.
- Obtain referrals and ensure timely education and signing of consents.
- Maintain effective communication with internal and external stakeholders.
- Coordinate public education and communication efforts to increase community outreach.
- Represent the company at various community and business meetings.
- Consistently promote company values.
Job Requirements:
- Bachelor's Degree from an accredited university.
- Minimum of 2 years' experience in referral development, marketing, or sales in healthcare.
- Current relationships with healthcare facilities or medical professionals in the area.
- Excellent communication and organizational skills.
- Knowledge of hospice philosophy and criteria.
- Understanding of HIPAA guidelines and patient information confidentiality.
- Self-motivated and able to work independently.
- Proficiency in computer applications.
