We are seeking an experienced and dynamic Director of Clinical Service (DOC) to lead our clinical team and uphold our mission of delivering exceptional, customized services to our clients.
Position Overview: The Director of Clinical Service is a key leadership role responsible for overseeing the clinical operations of the agency. The DOC will manage and mentor the clinical team, ensure regulatory compliance, and lead quality improvement initiatives. This role requires a proactive and compassionate leader who is committed to providing tailored care solutions for seniors.
Key Responsibilities:
Leadership & Team Development
• Lead the clinical team, including RN Case Managers, therapists, and support staff, ensuring high-quality, compassionate, and personalized care.
• Oversee the hiring, onboarding, and continuous training of clinical staff, promoting a culture of excellence, empathy, and tailored service.
• Foster a collaborative and supportive work environment that aligns with the company’s mission of delivering customized care solutions for seniors.
Clinical Oversight & Patient Care
• Direct and supervise patient care services to ensure individualized, tailor-made care plans meet the unique needs of each client.
• Review and approve care plans, providing guidance to RN Case Managers on complex cases, chronic conditions, and specialized treatments.
• Conduct regular case conferences, focusing on proactive care strategies and personalized interventions that enhance patient outcomes.
Quality Assurance & Compliance
• Lead quality improvement initiatives, ensuring that Your Tailor Made Senior Service upholds the highest standards of care and meets all regulatory requirements.
• Ensure comprehensive and accurate documentation in electronic health records (EHR), supporting compliance with Medicare, Medicaid, and state regulations.
• Perform regular audits and chart reviews, identifying opportunities for improvement and implementing corrective measures.
Customized Care Planning & Advocacy
• Collaborate with patients, families, and caregivers to develop customized care plans that reflect the specific needs and preferences of seniors.
• Serve as an advocate for patients and families, ensuring they receive the support and resources needed for optimal health and well-being.
• Engage with community resources and healthcare partners to provide a seamless, integrated approach to senior care.
Policy Development & Strategic Planning
• Develop, update, and implement clinical policies that reflect the tailored approach of the agency, incorporating the latest best practices and regulatory requirements.
• Work with the executive team to plan and execute strategies for growth, focusing on expanding personalized services and enhancing client satisfaction.
Financial Management
• Collaborate with the financial team to manage clinical budgets, monitor expenses, and ensure the efficient use of resources without compromising the quality of care.
• Track and analyze staff productivity and resource utilization, implementing strategies to maximize efficiency and effectiveness.
Professional Development & Community Engagement
• Stay current with industry trends and home health regulations, continuously seeking opportunities for professional growth and development.
• Represent Your Tailor Made Senior Service at local events, professional meetings, and community activities to build relationships and promote the company’s tailored services.
Qualifications:
• Current RN license in Texas.
• Minimum of 3-5 years of experience in a home health leadership role, preferably as a Director of Nursing or Clinical Manager.
• Strong knowledge of Medicare, Medicaid, and state home health regulations.
• Proven ability to lead and manage a clinical team with a focus on quality improvement and patient-centered care.
• Excellent communication, organizational, and problem-solving skills.
• Passionate about providing personalized care and advocating for seniors.
