The Manager, Home Health Intake is responsible for the overall coordination of Agency referrals and incoming potential patients to determine the ability to meet needs. They work within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules and goals.
+ Assesses and records all patient information, identify payer sources, determine eligibility, and notify branch of referral to ensure seamless transition of care.
+ Monitors the process/workflow within the department to identify gaps and creates action plan to meet desired outcomes.
+ Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area.
+ Requires cross departmental collaboration, conducts briefings and area meetings; maintains frequent contact with other managers across the department.

+ Bachelors degree or equivalent generally required.
+ Diploma, Associate or Bachelor Degree in Nursing preferred.
+ 5 or more years of leadership experience preferred.
+ Operations experience; Home Health operations experience preferred.
+ Experience with patient referral/intake process preferred.
+ Homecare Homebase (HCHB) system experience preferred.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$84,600 - $116,300 per year
