The LVN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required. We are open for PRN Role as well.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- The essential functions include, but are not limited to the following:
- Provide skilled nursing care under the direct supervision of the Clinical Supervisor
- Complete evaluation tasks, including vital signs and medication reviews
- Coordinate with physicians, therapists, and other individuals in the client’s care plan
- Assess and chart observations of the client’s condition at each visit
- Maintain complete and accurate documentation of nursing care
- Work collaboratively with all members of the healthcare team
- Provide education to clients and families on proper home health care procedures and strategies
- Record patients' medical information and vital signs.
- Monitor, record, and report symptoms or changes in patient's conditions.
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans
- Modify patient treatment plans as indicated by patients' responses and conditions
- Monitor all aspects of patient care, including diet and physical activity
- Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems
- Perform other duties as assigned
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
- Associate degree required; Graduate of an accredited state-approved, Bachelor’s Degree from the school of nursing preferred
- Current and valid California Licensed Vocational Nurse license
- One year of LPN experience, Current CPR certification (required), Driver’s License, and Car Insurance. Car Registration
- Strong interpersonal skills that build personal rapport and establish trust with the client
- Computer literate, Strong organizational skills
- Excellent written and communication skills
- Ability to meet physical demands of the job (e.g.: lifting 50 pounds for transfers)
- Home health care experience
- Bilingual (Spanish) preferred
