SUMMARY:
The Licensed Practical Nurse is responsible for the delivery of patient care through nursing processes of implementation and evaluation; under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Works in accordance with agency philosophy, standards of professional nursing practices, and all federal and state laws and regulations, including HIPAA as applicable.
What you will be doing:
- Sets up medications for clients and monitors side effects, drug interactions, dosages, and implements changes per physician instructions. Informs client and family of changes immediately.
- Listens, documents and reports significant changes in condition of clients, including incidents, unexplained injuries, medication errors, or evidence of client or family dissatisfaction to Clinical Director and/or RN Case Manager.
- Provides direct patient care as defined in the state Nurse Practice Act.
- Implements the appropriate aspects of the strategy of care and follows the plan of care initiated by the registered nurse.
- Contributes to the patient assessment and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
- Participates in quality assessment and assurance activities including implementation of recommendations and monitoring of results.
- Maintains ongoing communication with other disciplines to evaluate client responses to interventions, suggest alternative approaches when indicated and to revise and update plans of care to reflect progress toward goals.
- Facilitates and supports inter-disciplines communication to meet clients’ needs.
- Immediately informs the client, consults with the client’s physician and notifies the designated family member and/or the client’s legal representative when there is an accident involving an injury which as potential for requiring physician intervention, a significant change in the client’s physical, mental, or psychosocial status, a need to alter treatment significantly.
- Reports changes to Clinical Director and/or RN Case Manager.
- Interacts with clients, family members, legal representatives, and resolves concerns.
- Attempts to resolve all client grievances seeking consultation or assistance from Clinical Director and/or RN Case Manager.
- Consistent application of infection control procedures and universal precautions for bloodborne pathogens, use of protective equipment, handling of hazardous materials, and any other OSHA mandate on workplace safety.
- Serves as Certified Home Health Aide when coverage is needed.
- Completes required documentation of care and services delivered during shift including subjective findings, objective symptoms, interventions, and client responses to interventions per agency policies and procedures and federal and state regulations.
- Reports relevant information to the Clinical Director including interdisciplinary team functioning and communication, regulatory compliance issues, client risk factors, sudden changes in client behavior, expressed dissatisfaction by client or family, refusals of treatment, unexplained injuries, staff behavior (positive and negative), staffing needs, equipment and supply needs, opportunities to revise policies, procedures, and improve quality.
- Comply with the agency’s privacy practices and procedures related to client and employee records and all state/federal health privacy laws as outlined by HIPAA.
- Demonstrates C.A.R.E. values to our clients, family members, customers and staff.
- Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided, used, and maintained.
- Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services are provided to the clients we serve.
Job Requirements:
- Current Licensed Practical Nurse license in Indiana
- Previous home health &/or long-term care experience preferred
