Our Award-Winning Client is seeking a compassionate Home Health Licensed Practical Nurse (LPN) to join their extraordinary care team in Harrisburg, PA. The ideal candidate will possess an Associate degree, an LPN license in the state of Pennsylvania, and a minimum of two years of LPN experience. This role involves providing patient care plans, supporting patients in the Harrisburg area, and demonstrating proficiency with technology tools.
Key Responsibilities
- Collaborate to provide management and delivery of patient care plans.
- Support patients in Harrisburg, PA, and surrounding areas.
- Meet critical deadlines for charting.
- Self-manage time and schedule efficiently.
- Demonstrate experience with tablets, mobile phones, and EMR software.
Qualifications
Required:
- Associate degree or higher.
- 1+ years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR systems
- TruChart EHR
- Valid Licensed Practical Nurse (LPN) in the state of Pennsylvania (PA).
- ANY of the following valid licenses/certifications:
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Basic Life Support (AHA BLS)
- Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance.
Preferred:
- Previous Home Health/Hospice experience, any type of acute care, or LTC care.
- LPN License Number.
Schedule and Shift Details
- Monday-Friday, 8am-5pm, with one weekend every 4-8 weeks (Saturday & Sunday, 8am-5pm).
- Work schedule and shift expectations will be provided during onboarding.
