Home Health Medical Social Worker - American River Healthcare

**Job Summary:**
We are seeking a Home Health Medical Social Worker to provide support to patients receiving home health care services. The ideal candidate will assist patients and their families in coping with social, emotional, and economic issues stemming from their medical conditions.

**Qualifications:**
- Bachelor's or Master's degree in Social Work
- Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred
- Previous experience in medical social work or home health care
- Excellent communication and interpersonal skills
- Strong knowledge of community resources and support services
- Compassionate and empathetic approach to patient care

**Responsibilities:**
- Conduct psychosocial assessments of patients and their families
- Develop and implement care plans to address social and emotional needs
- Coordinate with healthcare team members to provide comprehensive patient care
- Advocate for patients to ensure access to necessary resources and support services
- Provide counseling and emotional support to patients and families
- Maintain accurate and timely documentation of patient interactions and progress
