Qualifications

Associate degree or higher

1+& years of experience in& ANY& of the following:

Electronic Medical Record (EMR)

Other EMR

TruChart EHR

1+& years of experience in& ANY& of the following:

Acute Care

Rehabilitative Setting

Valid Physical Therapy Assistant in the state of Illinois (IL)

Valid Regular Driver License (DL)

Is your degree in Physical Therapy? (yes)

Have you applied to or worked for Residential Home Health Hospice anytime in the past 6 months? (no)
