POSITION SUMMARY:

We are seeking a Physical Therapistto:

• Join our world-class team of doctors, nurses and other health care professionals in providing everything from preventive health care to highly specialized programs.
• Provide quality care in a compassionate patient- and family- centered environment.
• Plan, evaluate and administer physical therapy services and care to patients per physician's orders in their home on an intermittent basis according to the policies of the Agency.
• Direct physical therapy assistants in providing treatments to clients following Agency guidelines.
• Participate in continuous quality improvement activities and educational experiences in support of departmental philosophy and objectives, as well as, Health System initiatives.

AGE SPECIFIC CARE:

Provides age and culturally appropriate assessment/screening, interpretation of clinical and laboratory data and develops and implements age appropriate interventions and plans of care including education within the parameters of his/her position responsibilities and licensure.

EDUCATION/TRAINING:

Graduate of an accredited Physical Therapy program.

LICENSURE/CERTIFICATION:

• Current license from the State of Illinois.
  • An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment.
  • An applicant who fails to proved this documentation will not be hired.
  • A new employee will not be allowed to work as a therapist or assistant until evidence of licensure eligibility is provided.
• Bi-annual CPR certification required.
• Valid state driver's license and proof of liability insurance.

EXPERIENCE/SKILLS/ABILITIES:

• Ability to effectively communicate orally and in writing in a caring, considerate and concerned manner.
• Willingness to accept responsibility.
• Ability to direct ancillary staff and organize work assignments.
• Leadership skills essential to the position.
• Knowledge of JCAHO, CMS, IDPH rules and regulations preferred.
• Successful completion of annual clinical, age and job specific competencies and skill verification tools required.
