Home Health RN Clinician 2 - University of Virginia Health System

The Home Care RN Clinician 2 is an experienced capable clinician focused on expanding knowledge and skills. Consistently provides effective direct care to patients in their homes for an assigned caseload of patients; as part of the interdisciplinary team; oversees the care planning and care coordination for a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients their family physician, and other members of the health care team and in compliance with homecare standards and regulatory requirements. The Home Health nurse seeks, as well as provides feedback for improved clinical practice. Assumes a beginning leadership role but seeks mentoring in this process.

Position Compensation Range: $82,992.00 $110,240.00 Annual

UVA Continuum Home Health Care offers our nursing clinicians autonomy in their professional practice, provides scheduling support while also allowing self scheduling and provides mileage reimbursement at the Federal level. MINIMUM REQUIREMENTS

Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.
Experience: 1 year of Relevant Experience
Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver?s License required. American Heart Association (AHA) Health Care Provider BLS certification required.
