About The Position
**Job Title:** Registered Nurse I **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** No guaranteed hours **Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience **License:** Must have a license in hand **Certifications:** CPR, BLS **Must-Have:** Proficiency in HomeCare HomeBase (HCHB) and OASIS within the last year **Job Description:** - Provide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures. - Responsible to the Clinical Manager and directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Perform home visits and attend periodic office meetings; must have reliable transportation, a current driver's license, and agency-required liability insurance. - Average of 5-8 patients per day; contractors will be assigned between 5-10 patients per day that equate to roughly 6-8 points. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Must be willing to travel up to 75 miles from the home clinic. Mileage reimbursed at the IRS standard rate for travel between patient homes. - Proficient with HomeCare HomeBase (HCHB) and recent use within the last 6 months is required, with retraining if used outside of this window. - RNs, PTs, and OTs are required to be proficient in OASIS. - Must complete all documentation within 24 hours of the patient appointment. - The clinician will accept every patient on their schedule, with no declines based on distance. - Weekly productivity expectation is 30 points equating to 40 hours per week. - Must float to nearby clinics if not receiving enough patients to meet financial needs. **Note:** First-timers not accepted; must have recent specialty experience. Local applicants accepted at the same rate. Weekend and on-call requirements vary by location. The dress code is scrubs.
