About The Position
**Job Title:** Registered Nurse - Home Care **Job Specialty:** Team 5 - Northwest Philadelphia and Montgomery County **Job Duration:** 11 months **Shift:** 8:00 AM - 4:30 PM **Guaranteed Hours:** 40 **Experience:** Minimum years of experience required **License:** Registered Nurse license **Certifications:** Not specified **Must-Have:** Availability for shifts and guaranteed hours **Job Description:** - Provide comprehensive nursing care to patients in the Montgomery County area, including but not limited to Collegeville, Norristown, Royersford, Limerick, and Gilbertsville. - Collaborate with interdisciplinary teams to develop and execute patient care plans. - Monitor patient conditions, document observations, and report any changes to the healthcare team. - Administer medications and treatments as prescribed by healthcare providers. - Educate patients and their families on health management and disease prevention. - Ensure compliance with healthcare regulations and standards. - Maintain up-to-date patient records and documentation. - Participate in staff meetings and ongoing training sessions. - Exhibit excellent communication and interpersonal skills. - Flexible and capable of adapting to different home environments and patient needs.
