Our Award Winning Client is seeking a compassionate and experienced Registered Nurse (RN) to join their extraordinary care team. This full-time position involves providing management and delivery of patient care plans in California, PA, and surrounding areas. The role requires on-call availability on weekends and local travel to patient locations. The successful candidate will have an associate degree, an RN license in the state of work, and at least one year of RN experience.

Key Responsibilities:

• Lead and collaborate to provide management and delivery of patient care plans.
• Support patients in California, PA, and surrounding areas.
• Spend ~20% of the day driving to/from patient locations.
• Consistently meet critical deadlines for charting.
• Self-manage time and schedule.

Qualifications:

Required:

Associate degree or higher.

1+ years of experience in ANY of the following:

• Clinical Documentation.
• Clinical Informatics.
• Clinical Quality.

1+ years of experience in ANY of the following:

• Electronic Medical Record (EMR) systems.
• Practice Partner EMR or other EMRs.

Valid Registered Nurse (RN) license in the state of Pennsylvania (PA).

All of the following valid certifications:

• American Red Cross Advanced Life Support (ARC ALS).
• American Heart Association Advanced Cardiac Life Support (AHA ACLS).

Preferred:

• Please provide your RN License number.

Schedule and Shift Details:

• Schedule: Friday, Saturday, Sunday on-call from 5:00 PM to 1:00 AM.
• Scheduled to see patients during the day on Saturday/Sunday (8 units per day).

Travel:

• Local travel requirements, with a 20-25 mile radius surrounding the posted location.

Top Experiences:

• ER/Med surg hospital experience.
• Prior home health or hospice experience.

Top Misses (Experiences Not Desired):

• Extended time in skilled nursing facilities (SNF), dialysis, private duty nursing, or other limited-scope settings.
