Job Details:The Infusion Pharmacist is responsible for providing pharmaceutical services to patients receiving infusion, specialty or retail services in a manner that maximizes quality and patient safety. This position participates in and oversees the dispensing of medication in accordance with company, state and federal policies, laws and regulations. TPN and pediatric experience preferred.
Essential Job functions, duties, and responsibilities:
- Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for therapy changes to physicians and healthcare practitioners as appropriate.
- Maintain patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies as appropriate).
- Collaborate with the healthcare team to provide clinical and technical support as needed
- Document all communication with physicians, nurses, other members of the healthcare team and patients and their family members in the medical record.
- Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy.
- Implement policies and procedures for the safe procurement, storage, distribution and use of drugs and biological, as well as disposal of drugs.
- Assist healthcare personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs.
- Provide one-on-one drug therapy consultation with patients to identify, prevent and resolve drug therapy problems including inappropriate, ineffective, unsafe therapy and/or issues related to compliance.
- Prepare drug therapy care plans with recommendations for the patient to share with the physician. Review therapy plans with physician when warranted.
- Follow up with patient to monitor drug therapy and to promote positive outcomes.
Licensure, Certification, Experience and Education requirements:
- BS Pharmacy or Pharm D - Graduate of ACPE accredited School of Pharmacy.
- Current licensure in the state of NY.
- One-year experience in home infusion, hospital or managed care setting preferred.
- IV clinical experience preferred.
- Skill in use of electronic data management and computer systems
- Strong clinical and analytical problem-solving skills
- Outstanding telephone skills and etiquette
