Required Qualifications:
- An Associate degree or higher-level qualification
- A minimum of 3 years in roles involving Inpatient Facilities or Medical-Surgical environments
- Current and Valid Pennsylvania Registered Nurse (RN) Certification/License
- Holder of American Red Cross Basic Life Support (ARC BLS) or American Heart Association Basic Life Support (AHA BLS) certifications/licenses
- Appropriate Driver's License, Reliable Vehicle, and Auto Insurance
Ideal Candidate:
Our preferred candidate will be a Registered Nurse who brings prior experience in home health care to the role.
Overview:
As a Home Recovery Care Nurse, you will deliver top-tier care, equivalent to hospital standards, right at the patient's home as determined by the Admitting Provider's Treatment Plan. You will actively attend interdisciplinary care teams, deliver superior care, and participate in patient-focused recovery support in the home setting.
Key Responsibilities:
- Active participant in the Admission Huddles with the Admitting Provider and Recovery Care Coordinator
- Verify Admission Health and Home Assessment conclusions during the initial visit to patients' homes
- Implementation of physician's treatment plan, reporting any noticeable changes
- Charting and assessing patients' conditions during home visits
- Deliver nursing services such as comprehensive patient assessment, IV initiation and site care, basic wound care, lab sample collection, patient education, and catheter care, among others, in the patient's home
- In-depth patient education on correct home health care strategies and procedures
- Preparation and submission of a clinical report that delineates services offered and patient's progress towards set goals
- Timely communication of any changes in physician orders to the Recovery Care Coordinator
- Prompt reporting of any patient changes to the Admitting Provider and Recovery Care Coordinator
- Activation of Escalation Protocol for significant changes in patient condition or status
- Contribute to the discharge planning process, identifying any needs within the care plan
- Supervise patient care, instructing the patient or responsible caregiver in their role
- Daily virtual meetings with the Admitting Provider and patient to discuss progress and care plan
- Educate patients on the operation of the telehealth system kit
- Demonstrate time management and critical thinking capabilities at all times
Schedule:
Monday - Thursday 11am - 9:30pm (Four 10-hour days)
