Job Title: Home Visit Nurse Practitioner

Location: Bronx, NY

We are seeking compassionate and dedicated Nurse Practitioners to join our team as Home Visit Nurse Practitioners in Bronx, NY. In this role, you will provide primary care services to patients in the comfort of their own homes.

Responsibilities:
- Conduct comprehensive assessments of patients in their homes
- Develop and implement individualized care plans
- Provide health education and counseling to patients and their families
- Perform physical exams and order diagnostic tests as needed
- Administer medications and treatments as prescribed
- Coordinate with other healthcare providers to ensure continuity of care
- Document all patient interactions and treatment plans in electronic medical records
- Participate in team meetings and case conferences

Qualifications:
- Current Nurse Practitioner license in the state of New York
- Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
- Minimum of 2 years of experience in primary care or home health
- Excellent clinical assessment and diagnostic skills
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Bilingual in English and Spanish is a plus
