This position is for full-time or part-time, day shift, Monday through Friday.
Guardian Angel Hospice desires a Hospice Aide to join our growing team! The ideal applicant will be able to provide our patients with routine activities of daily living in accordance with the patient’s Plan of Care and as directed by the RN Case Manager. The ideal applicant must have the capability to effectively support, comfort, and educate patients, families, and others while maintaining confidentiality.
The Hospice Aide:
- Provides direct patient care to patients under the direction of the RN to ensure optimum care, including but not limited to: Assists or performs bed bath, tub, shower, or sponge bath; performs shampoo in bed, tub, shower or sink; checks, cleans and files nails; performs skin care; assists or performs oral hygiene; assists the patient with their ADL's.
- Provides personal care for each patient in accordance with the Plan of Care.
- Contribute to developing the Hospice Plan of Care by reporting observations to the RN Case Manager.
- Observe, inform, and document changes in the patient's physical and emotional condition to the Interdisciplinary Team.
- Respect professional boundaries while providing care in facilities and patient homes.
- Demonstrate safe techniques in all transfers and ambulation in the following area: Utilization of hoyer lift, wheelchair, and gait belt when transferring patients; safe and correct use of assistive devices including walker, cane, and crutches.
- Provide emotional support to patients and caregivers.
Job Requirements
- Must have reliable transportation, a valid driver’s license, and willingness to travel within the office’s service area.
- Current CNA (Certified Nursing Assistant) and/or HHA (Home Health Aide) Indiana State certification.
- Willingness to serve on the on-call rotation schedule.
- The ability to perform the physical duties of this position.
- Ability to manage your schedule effectively and efficiently.
