Our client, a large health care system in NYC, is seeking an Access Center RN.
Responsibilities:
- Evaluates patients referred for Hospice and/or Palliative Care services.
- Manages and facilitates the transition of patients in hospice or palliative care in accordance with applicable law, regulations and agency policies and procedures.
- Provides clinical support, leadership and oversight for growth and development of community and facility based hospice programs.
- Duties include planning, delivering customer service, development for service area expansion, coordination of internal and external clinical marketing efforts, community outreach, education, and clinical service/support for hospice patient/family direct care.
- Determines eligibility and level of care
- Communicates discharge plan verbally and in writing to referring MD and Discharge Planner
- Meets with patients and families, and obtains signed consent
- Arranges discharge, transportation
- Provides report to Admission Nurse and IPU IDT Staff
- Assesses needs for DME, supplies, medications
- Assists with completion of admissions as requested and neede
Requirements
- Graduate from an accredited School of Nursing; BSN preferred.
- A minimum of two years experience in either Hospice, Palliative, Critical or End-of-Life care preferred.
- New York State RN license, CPR-BLS and valid NYS Driver's license.
- Microsoft Office, Word and Excel.
- Ability to effectively communicate in a customer service environment.
- Ability to work flexible hours to meet the needs of the position.
- Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
- Possess teaching, communication, and listening skills.
- Ability to work non-judgmentally with patient/family members of any culture, religion, socio-economic background or lifestyle.
- Ability to speak in public.
- Ability to work within an integrated health care team.
