• Conduct thorough assessments of patients' medical conditions, symptoms, and pain management needs in the hospice setting in Edison, NJ.
• Develop personalized care plans in coordination with the hospice interdisciplinary team and the patient's family.
• Regularly update and review care plans to adapt to changing patient needs and maintain the highest quality of care.
• Administer skilled nursing care, including wound care, medication management, and symptom control for patients in Edison, NJ.
• Monitor patients' vital signs, treatment responses, and overall well-being to ensure optimal care outcomes.
• Collaborate with healthcare professionals like physicians and palliative care specialists to enhance patient comfort and pain management.
• Offer emotional and spiritual support and counseling to both patients and their families during the end-of-life process.
• Respect and honor patients' spiritual beliefs and cultural practices to foster a sense of peace and acceptance.
