The Hospice Case Manager RN will collaborate with a multidisciplinary hospice team to ensure effective patient care and desired outcomes, considering patients' age, gender, and cultural diversity. Responsibilities include supervising Hospice aides, providing on-call coverage, and contributing to quality nursing care through performance improvement processes.

Candidates must have a Georgia RN licensure, current BLS certification, and an Associate's degree. A minimum of one year of clinical nursing experience is required, with a preference for those with hospice experience and CHPN certification. Strong clinical skills, excellent customer service, and the ability to work independently and as part of a team are essential.
