Job Description:
As a Certified Nursing Assistant (CNA) with Superior Hospice, you will play a vital role in delivering personalized and compassionate care to patients in their homes. You will work closely with our interdisciplinary team to support patients and their families during a sensitive and challenging time.
Responsibilities:
- Provide direct patient care under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Administer medications and treatments as directed by the RN or LPN.
- Monitor and record patient vital signs and report any significant changes to the nursing team.
- Assist in maintaining a clean and safe environment for patients.
- Provide emotional support and companionship to patients and their families.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Participate in team meetings, case conferences, and in-service training sessions as required.
- Adhere to all company policies, procedures, and regulatory requirements.
Qualifications:
- Valid Certified Nursing Assistant (CNA) certification in the state of Minnesota.
- Previous experience in hospice, home care, or a related healthcare setting preferred.
- Compassionate and empathetic demeanor with excellent communication skills.
- Ability to work independently and as part of a collaborative team.
- Strong organizational skills and attention to detail.
- Reliable transportation and a valid driver's license.
- Ability to pass a background check and drug screening.
