The Patient Care Coordinator is responsible for timely filing and processing electronically of patient records to patient chart, copying, tracking, and obtaining Physician signatures, obtaining medical records. Prepare agenda and organize documents for the IDG meeting. The overall scheduling and tracking of discipline visits (i.e., Hospice Aides, LPN, RN, MSW, CH, etc.) under the supervision and oversight of the Clinical Director/Manager to assure appropriate staffing levels and visit frequency of all field are always met. Ensure that each patient record is closed at the time of patient discharge. Assist Clinical Director/Manager in obtaining signatures.
ESSENTIAL POSITION FUNCTIONS
• Creates and communicates staffing schedule that always assures adequate staffing per set guidelines.
• Schedules Recertification and Comprehensive Assessment visits for the RNCM.
• Adjusts staffing schedule and communicates as necessary to compensate for call-ins, leave of absences or any other unforeseen circumstances.
• Completes miscellaneous clerical projects i.e., Data entry, filing, general office support, obtaining signatures from physicians.
• Demonstrates ability to prioritize and manage several responsibilities simultaneously in a timely fashion without compromising quality such as accuracy, neatness, and thoroughness.
• Answers telephone calls and/or visitors in a warm, sincere, and timely manner. Completes admission follow up calls, calls to home patients to check supply needs.
• Effectively communicates timely completion of individual employee schedules within established time parameters and in consideration of employee productivity expectations.
• Keeps track of certifications, re-certifications, physician orders, assessments as required.
• Assists with IDG (Interdisciplinary Group) preparation.
• Completes and sends discharge (transfer, death, revocation, moved out of area) paperwork to attending physicians, facilities, etc.
• Collaborates and coordinates with Office Manager and Clinical Director any/all requests for paid time off (PTO), vacation, maternity leave, Family Medical Leave Act (FMLA), and unpaid leaves of absence.
• Assures sufficient availability of admission packages, admission binders/folders for staff’s use.
• Recommends new approaches and effective scheduling practices to affect continual improvements in efficiency.
• Coordinates ordering and pick up of equipment.
• Orders medical supplies and ensures proper PAR levels. Maintains supply room for appropriate supplies and disposes of expired supplies.
• Handles inquiries and questions regarding various scheduling needs to ensure quick, equitable and courteous resolution.
• Complies with privacy practices/procedures and all state/federal privacy laws as outlined by HIPAA related to patient/client and employee records, to include but not limited to information accessed through any web-based system and/or electronic medical record(s).
• Complies with hospice policies and procedures, laws and regulations, standards of practice and established
work rules.
• Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services
are provided to the patients we serve.
• Demonstrates the values/mission to our patient/clients, family members, customers, and staff.
EDUCATION & QUALIFICATIONS
• High school diploma or general education degree (GED); or one to three months related experience and/or
• training; or equivalent combination of education and experience.
• One (1) – Two (2) years prior healthcare or office experience and previous experience in Hospice or Home
Health Care preferred, but not required.
KNOWLEDGE, SKILLS, ABILITIES
• Ability to act calmly under stress, and of not being hasty or display unprofessional behavior towards staff.
• Ability to staff accordingly during staffing shortages and under stressful situations.
• Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write
simple correspondence. Ability to effectively present information in one-on-one and small group situations
to customers, clients, and other employees.
• Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
• Ability to effectively read, write, speak, and understand English.
CERTIFICATES, LICENSES, REGISTRATIONS
• State Certified Nursing Assistant (CNA) and/or Home Health Aide certification preferred, but not required.
