Hospice Per Diem Admission Registered Nurse - JourneyCare Hospice

JourneyCare Hospice is seeking Hospice RNs in Schaumburg, IL. New competitive salaries, with immediate openings available!

Qualifications:

• Hospice Experience Required
• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR Certification
• 1 year of Hospice experience, preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
• Must be knowledgeable of quality assessment and assurance procedure

HOURS: Weekends Fridays,Saturdays and Sunday - Covering PM SHIFTS: 8 hour shift 3p-11p, 10 hour shift 2pm-12am or 12 hours shift 12p-12a

What You’ll Do:

• Perform pre-admission evaluations for the appropriateness of services.
• Initiates the admission process by educating and informing patients/families of its services.
• Obtains informed consent
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
