Due to growth, we are seeking hospice case managers to see patients around either Walkerton or New Carlisle - and going towards South Bend.
Also a separate position which is a hospice on-call RN - Friday to Monday

Job Details
Hospice RN seeing patients in facilities and private residences.
What you will be doing:
- Coordinates care: Hospice RN case managers organize and oversee the medical, emotional, and practical care provided to patients in hospice, ensuring their needs are met comprehensively.
- Conducts assessments: They assess patients' needs, symptoms, and overall condition regularly to develop and adjust care plans accordingly, striving to enhance comfort and quality of life.
- Provides support: RN case managers offer support and guidance to both patients and their families, addressing concerns, providing education about the hospice process, and offering emotional support during difficult times.
- Collaborates with the team: They work closely with physicians, other healthcare professionals, social workers, and counselors to ensure a holistic approach to patient care, promoting effective communication and coordination among all involved parties.
Skills you need:
- RN license in state of practice
- Hospice experience highly preferred
