JOB SUMMARY
The registered nurse plans, organizes, and directs hospice care, emphasizing community health education and experience. As a professional nurse, you will utilize your nursing skills to provide comprehensive and compassionate care to patients and their families within their homes and communities.
DUTIES & RESPONSIBILITIES
Patient Care
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
- Provide professional nursing care using the nursing process
- Develop care plans based on nursing diagnosis and incorporate palliative nursing actions
- Administer medications and treatments as prescribed by the physician
- Assist patients with activities of daily living and promote self-sufficiency and comfort care
- Coordinate patient care as the Case Manager
Communication
- Maintain accurate and relevant clinical notes regarding patient's condition and care
- Communicate with physicians and other healthcare professionals to coordinate the care plan
- Teach patients and families self-care techniques
- Provide a safe environment for patients
- Work with the interdisciplinary group to meet the emotional needs of patients and families
- Attend interdisciplinary group meetings
Additional Duties
- Participate in on-call duties as defined by the on-call policy
- Ensure necessary equipment and services are available
- Supervise ancillary personnel and delegate responsibilities when required
- Maintain and upgrade professional knowledge and practice skills through continuing education
- Actively participate in quality assessment performance improvement teams and activities
