Position Overview:
Our growing Hospice Client is seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our team. The ideal candidate will have a deep understanding of end-of-life care, exceptional clinical skills, and the ability to manage a diverse caseload. The hospice case manager will not mind extensive travel as required to meet the needs of patients and families. As a Hospice RN Case Manager, you will be responsible for coordinating and delivering high-quality, patient-centered care to our hospice patients and their families. Must provide exceptional customer service at all times and be able to work well among a hard working team. Participation in the on-call rotation is a requirement of this position, until the census supports an on-call FT position.
Key Responsibilities
Patient Care Coordination:
- Develop, implement, and manage individualized care plans for hospice patients.
- Conduct comprehensive assessments of patients' physical, emotional, and psychological needs.
- Coordinate with physicians, family members, and other healthcare professionals to ensure seamless and effective care delivery.
- Participates in Interdisciplinary Meetings and documents appropriately as required
Clinical Duties:
- Administer medications, manage symptoms, and provide hands-on care as required.
- Monitor and evaluate patient responses to treatments and make necessary adjustments to care plans.
- Educate patients and their families about disease processes, care options, and symptom management.
- Admit new patients as appropriate
Documentation and Reporting:
- Maintain accurate and timely documentation of patient care and progress.
- Prepare and submit required reports and paperwork in compliance with hospice regulations and standards
On-Call Rotation:
- Participate in a rotating on-call schedule to provide after-hours support and emergency care as needed.
- Respond promptly to on-call inquiries and provide guidance or interventions as required.
- Document all on-call activities in the on-call log and within the HCHB EHR
Interdisciplinary Team Collaboration:
- Work collaboratively with the interdisciplinary team to ensure holistic and coordinated care.
- Attend team meetings, contribute to care planning, and support team members in delivering optimal patient care.
- Document IDG notes as required in EHR
Family Support:
- Offer emotional support and counseling to patients and their families throughout the hospice journey.
- Assist with grief and bereavement support as needed.
- Collaborate with other members of the IDG team as appropriate
Qualifications, Education and Licensure:
- Unencumbered Registered Nurse (RN) license in Georgia or Compact State .
- Bachelor's Degree in Nursing (BSN) preferred.
- Certification in Hospice and Palliative Nursing (CHPN) is a plus.
- Active and Current BLS Certification
Experience:
- Minimum of 3 years of nursing experience, with at least 1 year in hospice or palliative care preferred.
- Demonstrated experience in care management and patient-centered care.
Skills and Attributes:

- Ability to work independently and manage time effectively.

- Willingness to travel extensively to meet the needs of the patients and families
- Ability to chart electronically in the HCHB EHR
Physical Requirements:
- Ability to lift and move patients as needed.
- Ability to travel to patients' homes and other locations within the full-service area. Note travel may be extensive at times.
Work Environment:
- Hospice care setting, which may include patient homes, nursing facilities, and hospice centers.
- Flexibility to work evenings, weekends, and holidays as part of the on-call rotation.
