Salary: $54.00 - $60.00 per hour
Summary
The Hospice RN Case Manager plans and directs compassionate care, creating individualized plans and educating the community. This role involves patient assessments, team coordination, supervising aides, and adjusting care plans with the Clinical Manager. The RN educates families on end-of-life care, addressing their concerns with empathy. Strong clinical skills, effective communication, and resilience are key to managing multiple patient needs and sensitive situations.
Responsibilities
- Plan, coordinate, and deliver hospice care in alignment with patient needs and individualized care plans.
- Conduct initial and ongoing assessments, taking into account physical, psychological, and social factors affecting patients.
- Collaborate with interdisciplinary team members, including physicians, social workers, and hospice aides, to provide holistic care.
- Provide guidance and supervision to hospice aides, ensuring quality and consistency in patient care.
- Document patient progress and coordinate with the Clinical Manager/DPCS to adjust care plans as needed.
- Educate patients and families on end-of-life care options, addressing concerns with empathy and professionalism.
Requirements
- Graduate of an accredited school of nursing; current RN licensure in the state of California
- Minimum of 1-2 years of recent acute care experience in a clinical setting, with 1-2 years of experience in home health or hospice care preferred
- Basic Life Support (BLS) Hands-On CPR certification
- Strong observational, communication, and problem-solving skills
- Valid driver’s license, active auto insurance, and a reliable vehicle, as travel is required for home visits
