JOB SUMMARY: Theprimary pw-pose ofthis positionis tocoordinate theadmissions of new andreturningresidents fromhome,hospital, orotherhealthcare settings and tomaintainthe facility atits maximum capacity.The position also entails a marketing componentto hospitalsand otherpotentialclientele for the purpose ofpromoting long term, transitional, and exceptional care offeredby the facility.

JOB REQUIREMENTS:
1. Mustbephysicallyandmentallycapableofperformingroutinejobduties.
2. Be personable with residents, families,and staff ina professional and cooperative manner.
3. Must havecompassion, tolerance,and understanding for the residents of the facility.
4. Goodorganizational,planning,andmarketingskills.
5. Knowledge of hospitaldischargeplanning andpolicies as it relates to the long term care population.
6. KnowledgeofJCAHO,OBRA,lDPH,andHFSregulationsrelevantto admissions, discharges, and resident rights.
MAINDUTIES:
Supportthefacility'sphilosophyofcareandstrivetoachieveitsgoalsand objectives. Receive all inquiries regarding prospective residentsfrom healthcare facilities&privateindividualstodetermineanadmission dateortoschedule an appointment. Conducttoursofthefacilitytoprospectiveresidentsand/orfamilymembers. Completethepre-admissionassessment(inquiry)formandpresent informationregardingaprospectiveresidenttotheAdministratorand appropriatestaffforevaluationtodeterminefacilityandmedicalappropriateness. Informnecessarydepartmentsofthearrivalofnewresidentsandcoordinate theadmission process as outlined in the facility policy. Complete theadmissions packet and necessary documentation for admissions :
obtaina"DON",IDPHForm#2536,andOBRAFormforappropriateresidents. Securedepositifapplicablefromresidentortheresponsibleparty.
Determine Medicareeligibility,daysremaining,andcommunicate pertinent information tothe officestaff.
Completepat1softheMOSformrelevanttoadmissions. ObtaincopyofMedicare,SocialSecurity,insurance,andothercards. Coordinatelongtermadmissionswithareahospitalcasemanagersand socialworkers and submit tothe Administrator for final approval.
1.Completedailycensussheets on census and resident sourcing for the Administrator.
Facilitatedaily admissions meetingsand providedataregarding resident/familybackground, family involvement, resident behavior, payerstatus,etc.tostaff inattendance and coordinate room changes as needed. Maintain communicationwith hospitalsocial workers/dischargeplanners,families,and nursing staff to coordinate resident readmissions. Greetnewresidentandfamilyuponadmission. ObtainapprovalfromtheAdministratorforalladmissions,marketingstrategies,andprojects. Maintaincurrentdatabaseregardinghospitalsocialworkers,discharge planners, case managers, and discharge formats.
