GENERAL SUMMARY
Responsible for assessing adults and children/adolescents and their families to determine level of need and appropriateness of service in a hospital setting. The Hospital Liaison Manager is directly responsible for the performance and supervision of assigned assessors providing supervision and direction and program implementation. This position is responsible for coordinating direct care with community partners regarding program protocols and daily operations and will serve as the point of contact for operational escalation needs, during both normal business hours and after hours, and be responsible for providing reports and tracking performance measure outcomes. This includes organizing and monitoring the daily tasks to include scheduling appointments, monitoring referrals, assisting by triaging issues for urgent appointments and analyzing the system reports in order to give feedback about job performance. This position is responsible for providing detailed reports to the community partners and to STRAC’s Division Director to be presented at monthly Southwest Texas Crisis Collaborative (STCC) Steering Committee Meetings.
ESSENTIAL DUTIES & RESPONSIBILITIES

- Direct supervision of the Intake and Eligibility hospital liaison staff including the scheduling of Intake assessments with clinical and medical providers
- Assess department needs and manage the development of Hospital Liaison Policies and Procedures. Coordinate any changes to Business Units in conjunction with the Clinic Administrator
- Oversight of the documentation of intake/assessment using approved software.
- Oversight of the determination of eligibility for services using established State and Center
- Actively participates in the hiring process and annual employee performance appraisals
- Produce and analyze the number of completed intake assessment to optimize staffing levels
- Review and monitor performance target reports impacting intake activity as well as analyze the data and develop a plan of action if necessary
- Conduct on-site hospital supervision for liaison staff. Provide training, performance feedback and monitoring of intake assessment for quality and training purposes
- Assist the Clinic Administrator with managing day-to-day responsibilities ensuring adherence to agency, contract and compliance performance standards
- Serve as the contact person between Hospital Liaison Unit and the Center Business Units
- Triage referrals, hospital discharges and coordination of linkage to next level of care.
- Respond to or redirect telephone community calls for appropriate triage and information as needed for potential appointments and or referral to appropriate departments
- Oversee all Intake and Eligibility hospital liaison activities and ensure that assessments are scheduled according to protocol, and adequate number of patients are eligible for services in collaboration with Business Units provider needs
- Follow up on consumer complaints and seek appropriate resolution
- Maintain compliance of STRAC and other contracts as point of contact during normal and after hours; provide direct oversight of operations and functions; attend weekly meetings/ participate in daily calls; manage/track liaison training schedules; assist with facilitating of 7 day hospital discharge appointments as needed per contract requirements.

MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience
- Master Degree in a behavioral health related field (Social Work, Psychology, Marriage & Family).
- Previous supervisory experience required
Other Requirements- Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business
- Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies
PREFERRED QUALIFICATIONS
- Bilingual (English / Spanish)
- Licensed Professional Counselor, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist or PhD.
- Three (3) to five (5) years of progressively responsible work experience in management or supervision of services to population served.
- Two (2) to Three (3) years of experience in a general healthcare setting.
SUPERVISION
- Work requires supervising and monitoring performance for a regular group of employees (2 or more full-time employees) including providing input on hiring/disciplinary actions and work objectives/ effectiveness, performance evaluations, and realigning work as needed.

- Must be able to meet the physical requirements to complete SAMA and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
- Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care.
Compensation Package
Salary Range $65,723.00 – $98,585.00 Per Year
