Hospital to Homeless Service Coordinator - St. Patrick Center

The Position:

St. Patrick Center, a ministry of Catholic Charities, is hiring a Hospital Homeless Service Coordinator as part of the Hospital to Housing (H2H) team. This Coordinator will be working in both hospital and community settings to engage homeless and housing-insecure patients to identify and secure services for their physical, behavioral, and basic needs outside of emergency medical services. The wage range is between $20 to $25 an hour. The core working hours for this position are Monday through Friday 8:00 am to 4:30 pm.
