South Shore Hospital, a dynamic 374-bed community hospital located in Weymouth, MA, is the only designated Level II Trauma Center south of Boston. Job Description Summary
As a Hospital at Home Hospitalist, you will play a pivotal role in providing high-quality care to patients transitioning from our South Shore Hospital campus (including the Emergency Department and Medical/Surgical Units) to the comfort of their homes.
Key responsibilities include:
• Admitting patients to the Hospital at Home unit.
• Providing direct patient care.
• Working collaboratively with a multidisciplinary team.

Providers will initially train on-site, with remote work opportunities becoming available once proving competencies. A stable, HIPAA-compliant, high-speed internet connection is required for remote work, and providers should be available to return to the hospital for urgent clinical or staffing needs as necessary.

Job Qualifications
This position is a licensed independent practitioner credentialed through the Medical Staff Services Office.
- Minimum Education: Medical Degree from an accredited institution; MD/DO.
- Minimum Work Experience: Internship and Residency in Internal Medicine or Family Medicine from an accredited institution. BC/BE in Internal Medicine or Family Medicine.
- Required Licenses / Registrations: Physician License, unrestricted, from Board of Registration in Medicine, Massachusetts Controlled Substance Registration, and DEA Registration (with DEAx number) from US DOJ.
- Required Certifications: active ACLS certification.
Required Skills
In addition to requirements detailed above:
- Must be able to work a flexible schedule of varying days, weekends, and occasional nights and holidays.
- Must be fluent in English, both verbal and written.
- Must be able to learn how to use Epic EMR.
Essential Functions
This position assumes clinical duties within the Division of Hospital Medicine (DOHM) and reports directly to the Associate Medical Director for Hospital at Home. You will be required to be credentialed through the Medical Staff Office, including maintenance of necessary licensures. The following is an outline of the essential functions of the Hospital at Home Hospitalist.
- Participates in Hospital at Home (HAH) patient acquisition through partnership with the HAH Intake RNs
- Provides direct clinical care and medical decision-making for patients admitted on the HAH unit, including virtual rapid response; coordinating visits with a Nurse or community Paramedic when possible
- Develops appropriate plans of care and follow-up based on the interpretation and outcome of diagnostic assessments, laboratory tests, and physical examination findings
- Orders medications and writes prescriptions according to organizational and regulatory policies and procedures
- Participates in daily HAH Interdisciplinary Rounds (IDR)
- Consistently provides high quality and timely documentation
- Practices cost-effective medicine in an efficient manner, maximizing available resources
- Provides oversight of HAH Advanced Practice Clinicians
- Collaborate with Medical control for Mobile Integrated Health (MIH) when needed
- Discusses patient data with other physicians and healthcare professionals as appropriate
- Demonstrates expert clinical judgment skills to function in an independent role
- Performs virtual nighttime coverage as needed, on a rotating and equitable schedule
- Participate in clinical onboarding and training of HAH staff, new recruits, and other clinical staff new to the program.
- Provides clinical care as a member of the “brick and mortar: Hospital Medicine Team” as needed
- Performs additional duties and responsibilities as required by the division of Hospital Medicine
