Presbyterian Medical Group seeks a Nurse Practitioner / Physician Assistant to join our established Hospitalist practice in Albuquerque, New Mexico. We're all about well-being, starting with yours. Practice Highlights:
- 1800 hours required per year
- 12-13 shifts per month
- 12 hour night shifts
- Closed ICU setting with no procedure requirements
Responsibilities:
Summary:
The CNS/NP/PA serves as clinical resource and an independent practitioner who diagnoses and manages acute and chronic diseases on the inpatient unit. Proactively manages the care of patients across the continuum of care. Oversees the case management of complex patients
Responsibilities:
*Completes a comprehensive health assessment including an appropriate health history (chief complaint, history of present illness, past medical history, family history, social history, review of systems, and pain history) and physical exam
*Determines the need for and interprets the results of relevant screening and diagnostic tests.
*Demonstrates sound clinical judgment and diagnostic reasoning abilities to formulate a diagnosis by critically analyzing history, physical examination, and diagnostic tests.
*Modifies assessment techniques according to patients condition, culture, age, and stage of development.
*Effectively communicates health findings and/or the diagnosis of a health condition to the patient, discusses the prognosis and options for treatment for the condition, and then establishes a mutually acceptable plan of care with the patient.
*Manages acute and chronic health conditions by providing effective pharmacological and non-pharmacological interventions; assists patients in following the plan of treatment, and selects the appropriate best practice treatment.
*Prepares written prescription orders, medications, and controlled substances under the rules and regulations of the State of NM and the Federal DEA.
*Initiates timely and appropriate consultation, referral or collaboration with the hospitalist and/or members of the health care team.
*Determines the need for and implements health promotion strategies for patients considering specific age and cultural requirements in accordance with population health principles to reduce risk factors and prevent disease
*Demonstrates good interpersonal communication skills for patients of various ages and cultures, and also has the ability to develop appropriate and professional rapport with patients, families, and other healthcare team members.
*Maintains patient medical records as required by hospital and regulatory agencies.
*Performs other duties as assigned.
Credentials Required:
Minimum two to three years experience in an acute care setting required. Prior experience working in a hospitalist setting desirable. Additional Skills: Knowledge of professional medical practice and care to administer and evaluate patient care; knowledge of organizational policies, regulations, and procedures to administer care; knowledge of medical equipment and instruments to administer patient care; knowledge of common safety hazard and precautions to establish safe work environment; skill in applying and modifying the principles, methods, and techniques of medicine to provide on-going patient care, skill in maintaining medical records, skills in establishing and maintaining effective working relationships with patients, families, patient care staff, supervising physicians, medical community and referring providers. Ability to react calmly and effectively to emergency situations.
