ABOUT OUR AGENCY
In 1967, with cooperation from The Resources Exchange and the Arc of the Pikes Peak Region, the Cheyenne Foundation was created. This organization was the first residential training center for adults with developmental disabilities in Colorado Springs. In March of 1971, the Board of Directors purchased the 16-acre McLaughlin Lodge in Manitou Springs Colorado to allow for expansion of the agency. With this property, a pilot project was established to demonstrate that individuals with intellectual and developmental disabilities (IDD) could live independently in cabins with 24/7 staff care. Here, they developed activities of daily living and job training. On April 1, 1971, the Board of Directors changed the name to Cheyenne Village Inc. and became the first licensed provider approved service agency in the state of Colorado. Since that time, we have grown tremendously, serving individuals throughout El Paso County in various setting types, which include; Host Homes, independent apartment living, and nine staffed Cheyenne Village-owned homes. Our humble beginnings have evolved into one of the largest residential providers in Colorado Springs.
HOST HOME COORDINATOR:
Maintain a caseload of individuals receiving services in Host Homes. Host Homes are residents within our community in El Paso County that bring in an adult that is intellectual and developmentally disabled and care for them as a family member. We are responsible for monitoring the home and services provided by site visits and other contacts to assure quality and regulatory compliance is being followed. We provide training to Host Home Providers and oversight of individual client(s) finances.
JOB RESPONSIBILITIES:
- Ensures Division for the Intellectual and Developmental health and safety standards are met (Colorado Department of Health and Developmental Disabilities Services). Represent these areas during survey process.
- Maintains assigned caseload and serves as first point of contact for Host Home Providers.
- Assists with initial and ongoing Host Home Provider training.
- Conducts or assists with completion of Person Centered Planning processes, including annual reviews of PCP documentation.
- Maintains contact on routine and non-routine issues with Cheyenne Village departments: Nursing, Business, and Maintenance.
- Provides direct support/service to each person based on the BISSP/ISSP goals. Conducts at least one monthly on-site visits to host homes and host home residents.
- Reviews and ensures completion and quality of documentation i.e. T-logs, data collect, MARS, etc. Ensures host home providers complete incident reports, including procedures for mandatory reporting of MANE incidents.
- Monitors medications: accurate counts and monthly checks of medications and MARS. Assist with monthly medication audits.
- Monitors individuals receiving services finances by performing reviews of cash reserves and balancing of financial records at least monthly.
- Ensures monthly monitoring visits are completed on assigned caseload.
- Inspects residences for safety, fire exits, cleanliness, and maintenance needs. Reports and takes action on these concerns.
- Conducts fire drills with newly placed individuals within 7 days. Coordinates completion of ongoing fire drills, evacuation procedure review and obtains reports.
- Promotes person centered services including active listening to the needs and preferences for the individual. Ensures individuals needs are met in a timely manner.
- Assists with paperwork relating to individual receiving services; i.e. leases, HQS.
- Assists with respite placements, to include identification of appropriate respite host home matches to resident needs and respite host home training as needed.
- Manages difficult or emotional customer situations; responds promptly to customer needs; solicits customer feedback to improve service; responds to requests for service and assistance; meets commitments.
- Performs all other job duties as assigned.
WORKING CONDITIONS:
Normal office environment. During site visits will be in home settings with one or two individuals receiving services. Environment may become hectic and noisy. Flexibility and ability to change priorities a must.
PHYSICAL DEMANDS:
Normal demands of office and host home residential setting.
LICENSES:
Maintain current QMAP, CPR, and First Aid certifications. Certifications and training are paid for by Cheyenne Village.
COMPENSATION
STATUS: Non-Exempt
PAY TYPE: Hourly
The salary range is $21.01 - $26.53 per hour depending on experience and qualifications.

Maintains criminal and driving checks acceptable with State of Colorado, insurance carrier and Cheyenne Village, Inc. requirements. Equivalent combination of education and experience may be considered.
