Responsibilities: As a House Manager at Palmer's Home Care, LLC, you will be responsible for creating and maintaining a safe, supportive, and therapeutic environment in our residential homes, fostering growth and rehabilitation at all times. Your main role will involve providing modeling, support, and supervision for Direct Care Staff to ensure appropriate and individualized support for each person we serve.
Key Duties and Responsibilities:
- Train and support staff (direct care, documentation, etc.) to ensure the health and safety of each individual at all times.
- Collaborate with the Director/Degreed Professional Manager to set and track individual goals/objectives.
- Assist in managing client benefits for individuals in the home (e.g., social security paperwork, food stamps, Medicaid).
- Attend appointments with individuals, ensuring follow-up and scheduling within required timelines.
- Manage medication changes, ensuring appropriate documentation and Monthly Administration Records (MARs) for each individual.
- Assist with NAFs (Nursing Authorization Forms) requests and gather necessary information.
- Oversee daily data collection and ensure accurate and timely documentation.
- Handle medication and medical supplies ordering and ensure individuals' needs are met.
- Spend time with individuals weekly and complete required documentation.
- Maintain daily census records and submitted all monthly paperwork on time.
- Ensure compliance with DMH requirements for individuals' books in the home.
- Participate in hiring and training new staff, ensuring proper documentation and completion of training.
- Coordinate activities and outings, as well as the use of the van.
- Report any DMH Reportable events or problematic behaviors promptly to the Director/DPM.
- Conduct monthly safety checklists and drills as required by DMH.
- Foster an environment of respect and empowerment for individuals and staff, providing support and corrective action when needed.
- Manage staff scheduling and time-off requests, and handle call-ins and shift trades.
- Ensure requirements for visits by Community RNs are met and nursing delegations are completed promptly.
- Maintain an accurate personal items inventory for each individual on your caseload.
- Assist with payroll by ensuring timely submission of time sheets for hourly employees.
Qualifications:
- Pass a family safety care registry screening.
- Exhibit a strong work ethic and the ability to provide and accept supervision and feedback.
- Display reliability, punctuality, and flexibility in scheduling to meet individuals' needs.
- Possess strong interpersonal skills for interacting with individuals, teams, and the community.
- Capable of lifting over 50 lbs.
- Hold a valid driving license and be able to drive a van.
- Complete all required agency training and certifications within 90 days of hire.
