About the Job
PathPoint is seeking a full-time ECM Care Manager/Housing Navigator to join our Behavioral Health division in Santa Maria, CA. The position is community based/remote and will cover the following locations: Santa Maria, Guadalupe, Lompoc, Buellton, and subject to change to surrounding Santa Barbara County Cities. In this role, you will provide community-based case management for an assigned group of clients, who may be without a home, have substance use disorder, and or a mild/moderate mental health diagnosis. Additionally, ensure our services are holistic, client-centered, and trauma-informed.
Schedule: Monday-Friday or Tuesday-Friday, 40 hours per week with your choice of 5 8-hour shifts or 4 10-hour shift that falls between 7:00am and 6:00pm.
Compensation: Starting pay is $25.00-$28.00 per hour and the position's pay range is $24.00 to $29.00 per hour. The pay range is subject to change based on a variety of factors.

What You'll Do:
- Outreach and engage eligible members.
- Meet with clients as needed in-person at the following locations but not limited to client's home, doctor's office, social services or other community settings, etc.
- Monitor and verify clients Medi-Cal eligibility status and assist clients in maintaining Medi-Call coverage.
- Conduct assessments and create whole-person centered care plan. Provide supporting documents to determine interventions and services.
- Identify housing opportunities and collaborate with landlords, property management, housing authority, etc.
- Provide on-going services to assist clients in meeting goals, needs, and care plan.
- Serve as an advocate and liaison to connect members to supportive services.
What We're Looking For:
- Bachelor's degree in related field such as sociology, psychology, or social work and the following work-related experience preferred or a combination of a bachelor's degree or HS Diploma or equivalent and 2 years of the following work-related experience.
- Experience in a social service setting working with families and/or individuals with mental health diagnosis, developmental disabilities, substance use disorder, homelessness or at risk.
- Knowledge of case management planning and systems, care plans, assessments, and the ability to record data on excel spreadsheets. Knowledge of community organizations, resources and programs.
- Bilingual (Spanish/English) preferred.
- Good written and oral communication.
- Must have a valid California driver's license with satisfactory driving record.
